Signs Your Loved One's Wound Needs Professional Attention

By R R

You noticed a small sore on your mother's ankle last week. You cleaned it, applied antibiotic ointment, and covered it with a bandage. Standard procedure for a minor wound.

But a week later, it doesn't look better. It might look worse. And now you're wondering: is this normal? Or is this something that needs a doctor?

For seniors, the line between "minor wound" and "medical concern" is thinner than most families realize. Aging skin heals more slowly. Underlying conditions like diabetes compromise the body's repair mechanisms. And wounds that would resolve quickly in a younger person can become serious complications in an aging adult.

Knowing what to watch for can mean the difference between a simple office visit and a hospital admission.

Warning Signs That Require Prompt Medical Attention

The wound isn't improving after a week of home care. Healthy wounds show visible improvement within a few days — reduced redness, less drainage, beginning of new tissue formation. If a wound looks the same or worse after seven days of proper home care, something is preventing it from healing.

Increasing redness that spreads beyond the wound edges. Some redness around a wound is normal. Redness that expands outward from the wound — particularly if it forms streaks moving away from the site — suggests infection is spreading and requires immediate medical attention.

Warmth and swelling around the wound. A wound that feels noticeably warmer than the surrounding skin, or that develops new swelling, may be infected. This is especially concerning in seniors with diabetes or compromised immune systems.

Drainage that changes color or smell. Clear or slightly yellow drainage from a healing wound is normal. Drainage that becomes green, brown, or thick, or that develops a foul odor, indicates infection.

Increased pain. Pain that intensifies rather than decreases over time, or pain that develops in a wound that was previously painless, warrants medical evaluation.

Fever or general malaise. When a wound infection becomes systemic, your loved one may develop fever, chills, fatigue, or confusion. In seniors, confusion is sometimes the first sign of infection — before fever or other classic symptoms appear.

Black or dark tissue around the wound. Dark or blackened tissue (eschar) may indicate dead tissue that isn't receiving adequate blood supply. This requires professional assessment and potentially debridement.

What You Can Do While Waiting for the Appointment

Keep the wound clean and covered. Don't apply new topical treatments without medical guidance — some over-the-counter products can actually impede healing in certain types of wounds. Ensure your loved one stays hydrated and well-nourished, as the body needs protein and fluids to support healing. Document what you're seeing — when changes started, what the wound looks like, any accompanying symptoms — so you can provide clear information to the healthcare provider.

How Daily Care Prevents Wound Complications

Most wound complications in seniors aren't caused by the initial injury — they're caused by inadequate daily care between medical appointments. Wounds that aren't kept clean, dressings that aren't changed regularly, skin that isn't monitored for changes, and nutrition that doesn't support healing all contribute to wounds that escalate from minor to serious.

Geriatric Care Solutions' Healing Ally program provides daily wound care coordination — trained caregivers who monitor wounds, maintain proper hygiene, ensure adequate nutrition, and communicate changes to your loved one's medical team before small problems become big ones.

We are a care coordination provider. We work alongside your loved one's medical team to ensure daily care supports their clinical treatment plan.
